MAKE A REFERRAL

Straightforward referrals. Individual assessments.

If you’re supporting someone who may benefit from supported accommodation, send us the relevant referral information and our team will review their circumstances, support needs and suitability.

Who may be suitable?

Open Home Living provides accommodation for adults aged 18 and over. We work with people from a range of backgrounds, including people experiencing homelessness, people with mental health support needs, people in recovery from substance misuse and people leaving prison or involved with criminal justice services.

Being part of one of these groups does not automatically mean a placement will be suitable.

  • Have a housing need
  • Require housing-related support
  • Have needs within the scope of our service
  • Be able to live safely within the accommodation available
  • Present risks that can be appropriately managed

Each referral is assessed individually based on the person’s circumstances, support needs and the suitability of available accommodation.

Who can refer?

Referrals may come from relevant professionals and organisations. Self-referrals may also be considered where sufficient information can be provided to complete an appropriate assessment.

HOW IT WORKS

A clear referral process.

We aim to make referrals straightforward for professionals, organisations and individuals seeking appropriate supported accommodation.

01

Submit referral

Clear information helps the team consider the next step.

02

Initial review

We review the information and identify whether anything further is required.

03

Assessment

We consider support needs, risks, location and compatibility.

04

Accommodation matching

We match suitability against the proposed property and existing residents.

05

Placement

Where an appropriate vacancy is available, next steps are discussed.

06

Support begins

A support plan is developed and reviewed as the resident works towards goals.

What information we need

To assess a referral properly, we need an accurate picture of the applicant’s circumstances. Providing complete information helps us make safer and more appropriate placement decisions. Relevant information should not knowingly be withheld where it could materially affect risk or suitability.

  • Current housing circumstances
  • Housing history
  • Support needs
  • Relevant health or mental health information
  • Substance misuse information where relevant
  • Known risks
  • Offending history where relevant
  • Safeguarding concerns
  • Licence conditions, exclusion zones or relevant restrictions
  • Benefit or funding circumstances
  • Current professional involvement
  • Any information that could affect placement suitability

What happens next?

Risk does not automatically mean exclusion

Many people requiring supported accommodation have experienced complex circumstances. The existence of risk information is not automatically a reason to decline a referral. We consider the nature and level of risk, current circumstances, professional involvement and whether those risks can be managed safely within our service. Where they cannot, we will not proceed with the placement.

Placement suitability

We consider support needs, mental health, physical requirements, substance misuse risks, offending history, risk to self or others, licence conditions, previous tenancy difficulties and compatibility with existing residents. The purpose is not simply to determine whether somebody needs housing — but whether the proposed placement can work safely and constructively.

Submitting a referral does not guarantee accommodation.

Where we decide we cannot offer a placement, we aim to explain the reason. Applicants or referrers who believe a decision should be reconsidered can ask us about the appropriate review process.
