FOR REFERRAL PARTNERS

A dependable housing and support partner.

This page is for organisations looking to develop an ongoing referral relationship with Open Home Living. We work with professionals who need confidence that accommodation is available, referrals are assessed properly, risks are understood and residents receive appropriate housing-related support.

What partners can expect

We aim to make the professional relationship clear, consistent and focused on appropriate outcomes for the people you support.

Clear assessment

Every referral is considered individually.

Risk-informed decisions

Relevant risk information is considered before placement.

Managed accommodation

Properties are managed with safety, suitability and resident wellbeing in mind.

Structured support

Residents receive housing-related support linked to agreed needs and goals.

Professional communication

Where appropriate and lawful, we communicate with relevant professionals involved.

Move-on focus

Future housing and greater independence are considered throughout the placement.

HOW WE WORK WITH PROFESSIONALS

A clear relationship from referral to ongoing support.

Working together

Strong placements depend on good information and good communication. Where appropriate, we work alongside the professionals already involved with a resident rather than trying to duplicate specialist services. This can include liaison with probation, mental health services, local authorities, drug and alcohol services and other relevant organisations.

A placement can become more difficult over time. Where concerns arise, we aim to identify problems early, understand any underlying support need and work with the resident and relevant professionals where appropriate. Safety, resident welfare, the interests of other residents and the wider community remain central to decision-making.

Our approach

Our approach is built around suitability, safeguarding and structured support.

01

Suitability — assessed individually against accommodation and support requirements

02

Safeguarding — central to how we operate and reflected in day-to-day practice

03

Support planning — structured plans focused on the resident's needs and goals

04

Communication — keeping referring professionals informed at relevant stages

05

Reviews — progress reviewed to ensure the placement and support remain appropriate

06

Move-on planning — supporting the resident's next stage of independence
